As someone above said: Single Payor, here we come.

Because, yes, the vast majority of Americans will soon be without Health Insurance. Small-to-Mid-Sized companies are dropping their insurance plans, because they can't afford them. Under 50 employees (or if they can game the system to get there) and they're just dropping it. Over 50 employees, and many of them have done the math to find out that the penalty tax is less than providing insurance with all the new mandates.

Big companies are mostly keeping their insurance (mostly), because of some things they can do with insurance funding (fully funded v fully insured). But even there, they are trimming back and streamlining as much as possible.

They can't make the "tax" too high because per Roberts reasoning (I know, I know) the max amount ($2k) was still small enough so as not to be punitive and could therefore be logically considered a tax and not a penalty. Now I don't know if he drew any hard distinctions in terms of a threshold where a tax becomes a penalty, but I would guess that if it were much higher than it is then that line would be crossed.

I'm hoping they will not be succesful, but I think Obama made a calculated decision to go for Obamacare now in an attempt to disrput the industry and to get single payor a few years down the line. Again, from his perspective it wasn't a bad strategic move. He couldn't get single payor in 2009 when he had full control. He could have made modest reforms and tried to build on it, or he could have made a drastic change thinking he could push it through during his 2nd term. I think he miscalculated on his own effectiveness.

Obama made a calcualted gamble, and it may still pay off for him. I hope it doesn't.

What this does is completely eliminate the insurance companies, and brings the total healthcare spending down substantially by instituting the Death Panel to stop this ridiculous spending on worthless procedures.

Oddly, you've hit on the real problem. Right now, through Medicare and Medicade the incentive to the provider is just that - "to keep your ass alive". Not to cure you, not to make you better than you were when you came in, not to ease your pain and certainly not to allow you to die with dignity.

Their only incentive is to keep your ass alive and make sure the procedures have the proper code attached so that the billing goes smoothly.

70% of our healthcare dollars are going to less than 5% of the people in their last year of life. This needs to be brought under control.

The main one being that we're being distracted and divided solely so that the few who are in a position to profit by the country's demise can get a little more time to steal a little more before it all falls apart.

When you go to a new doctor, or to the hospital, one of the first things they'll have you do is provide your insurance information. They won't even give you "I'll be paying cash" as an option.

So if you check "no insurance" they'll helpfully direct you to the kiosk (or whatever) where you can put your information and a shiny new 20K/yr insurance policy will be sold to you. With the exception of service through the ER (even then: only possibly) if you elect not to have insurance, you will be electing not to receive service.

They would rather pay 1k/month in premiums for annual usage of 2k (see Massachusetts, where 1/6 of the population can't afford to use their goldplated insurance because the premiums are so high, but they don't drop insurance and pay the much lower penalties at all) than pay the 2k out of pocket.

Not necessarily. Hillary fought for the provision in single payer to outlaw care outside of the government paid system. That was a deal breaker and probably the primary reason the plan didn't get pushed through.

In a workable system, the government would provide care up to the limits established by the Death Panel. If you were 86 years old, an alcoholic and in need of a liver transplant and rich, once the government turned you down for the transplant you would be welcome to pay for it yourself.

Enterprising insurance companies will probably sell policies that map out very specific covered items in such a system. Whatever the outcome, it would be far less expensive than what we have now.

327 I've brought this up before. As a retiree I pay for my health insurance. Each year I get a letter about how much it will go up, usually the increase was $10-20 per month. In October I got a letter stating that it would go up $148 per month. More than 10 times the usual increase.

Someone has to foot the bill for the free loaders who have an option to pay a penalty to the irs instead of buying insurance, then after they have a major medical problem can demand insurance from my company. It's kinda like, you don't buy car insurance. Then you total your car, go to the insurance company, buy insurance, make them pay to replace the car you already wrecked, then don't renew your insurance the next year.

I hate that bastard Obama.

Yes, there are plenty of people who have no idea how insurance companies operate. Just as there are a lot of people who don't know that when you raise taxes on businesses, it is the public that ends up paying those taxes.

I have this particularly ignorant neighbor who had no idea that when you take out a thirty year mortgage on a house, you are paying almost nothing off on the principle (sp?) for the first several years. She never bothered to look at the amortization chart. It's not like a thirty year mortgage is an important decision or anything.

Unfortunately these people vote. And they vote for the pols who will give them free shit.

But if these people, who are over-using in their old age, are like most people I know, they spent money on health insurance for decades where they barely used the insurance. I was like that. When I was in my twenties and thirties, my employer paid health insurance premiums for me, and I made very little use of it. My doctor would get after me because I didn't go in for a check-up for 5 years at a stretch.

Other than a couple of biopsies, about all I use, in my sixties is a check-up every couple years, a yearly mammogram, blood work up every 2 years, and a colonoscopy every 10 years.

There is a lot of bitching here, but no solutions.

My preference is to go with the conservative icon F.A. Hayek's interpretation of the proper use of government - to do what it is impossible for the individual to do. He found it more than acceptable that the government use it's power and resources to provide coverage for truly insurable risks.

A single payer catastrophic system makes sense. If it is coupled with Individual Health Maintenance Accounts to pay for foreseeable risks, it is a conservative solution to a very real problem.

Having the government run the catastrophic end is the only way around setting the limits of care so that the system (and the country) don't bankrupt themselves. I understand that it's really compassionate to want to give every 90 year old smoker a new set of lungs on our dime, but it just isn't practical.

People who want unlimited care for everyone could contribute into a fund to piss away their money, but for the whole country, we need a realistic system.

It was the "public option" that was going to be the Trojan Horse for Single payer. After a few years of that, more and more people are on the Public Option, and then they go "Hey, if the PO is so great, why are we wasting all these resources on the other private plans?" At that point we transition to single payer by putting the rest of the US on the Public Option, and game over.

But Obama didn't do that. You have employer plans, the shitty exchanges, or zilch. No public option. There is no plan B. Just Obamacare or the looming implosion.

No, the problem is allowing people, who freely choose to not buy insurance, to buy the insurance after they have a major medical issue. Then putting the insurance company into the position of increasing my rates to cover the costs.

If you don't buy insurance, and set aside money, someone suggested 25% of income, to cover health costs out of pocket, I respect you. I knew a guy who was not particularly highly paid, who saved up the money to pay for his own Lasic (sp) surgery.

Freeloaders are the people who don't buy the insurance, then think someone else should pay the cost when they develop a medical problem.

Every individual is allowed to purchase any and all of the health care goods and services that he or she desires and can afford, either individually or in voluntary association with others (family, friends, churchmembers, trade guild associates, strangers), from providers or manufacturers who are voluntarily participating in the exchange of goods and services, including those providers who are willing to provide services charitably, while simultaneously eliminating the federal government from the healthcare market entirely.

If by "the government" you mean state or local government, okay. It's a Constitutional option.

But it is NOT "the only way" it could be done. If government would give more leeway to private insurers, they could charge you megabucks for a policy that includes such things as lung transplants at age 90. If you want such luxuries, you'd have to buy a fat rider.

The system needs to be at the federal level because they would be the final authority in court. A state limit is fine until someone says "That's my momma in there!" and sues to increase the age limit for lung transplants from 90 to 91. It either gets set for all or you spend ridiculous sums litigating every condition and age.

Patients/customers will opt of out health insurance and start just paying cash for healthcare services underneath $20,000 or so.



Doctors/healthcare providers will start opting out of the system
too, and setting up cash only (or check or Mastercard but not insurance)
clinics.



Patients and doctors will figure out that they like this arrangement.



The feds will start looking for ways to make this illegal.

You might find this interesting, I posted it at the very end of the morning thread in response to what someone else said about a possible way around the government dictated Bronze/Silver/Gold/Platinum plan structure:
2 problems with that: #1, you pay the fine even if insured if you
don't have a "qualified" plan, no plans bought since 2010 are eligible
to be grandfathered in.
#2, They tried that in Massachusetts, which has
the same "qualified plans" rule. A national insurance company came in
and started marketing unqualified plans, basically $5000K deductible
major medical plans. They marketed them as "unqualified", and part of
the paper work required to sign up was acknowledging that buying this
plan meant one would still have to pay the "uninsured" fine. Still, the
cost of the plans plus the fines was many thousands of dollars
(sometimes $10K or more) less than what "qualified plans" cost, and
there was HUGE demand for these policies. The Mass. AG went after then
with their huge lawfare teeth bared for basically "Defying the will of
the people of Mass. as expressed through the duly elected legislature"
and drove them out of the state. Look for insurance companies to try and
stave off bankruptcy by offering "accident only" plans when January
rolls around, and look for the same thing to happen to them.

434 The CNSnews item is mixing concepts: mininum premium payments versus maximum penalties for failure to sign up. It's $20,000 IF a family of 5 at a total annual income of $120k forgoes signing up.

It's half that IF the same family of 5 signs up. The $2k figure applies to individuals, so: 5 people x $2k per person = $10k (more precisely, $10,425).

So what's CNSnews doing? It's reskewing a skew job by the IRS. The IRS aimed to publicize how NOT signing up "is" more expensive, when actually it's more like "may be" more expensive. CNSnews took that misleading mess and converted into an entirely different misleading mess.

What's the REAL effect of paying versus not paying? It depends: for low income types with dependent children, it'll be a lot less; for high middle income types with employable but unemployed slackers, it'll be closer to what the IRS is trying to scare folks over.

Reality: no one CURRENTLY paying for private health care coverage is going to see any short term reduction, and it's too early to tell about any middle or longer term because ... in the long run we're all dead. Meanwhile, those NOT currently paying for private h.c. coverage are going to be penalized, but circumstances are going to dictate how much and when if ever the penalty gets assessed.

I can see the motivation behind the IRS trying to sell the idea of 'just pay for it, fer chrissakes', in part because the IRS simply doesn't have the enforcement ability; but this is skewing by CNNnews is inevitably going to form part of the price government agencies have to pay for screwing around by deliberately misleading da folks.

Exactly. I have several medical conditions, andso Isprang for theRolls-Royce of a plan through my employer (who has a close enough relation with medical education and practice in the US that it's unlikely we'll ever drop coverage). For my husband and myself, I pay approximately $7K a year in premiumsfor my coverage, which includes virtually no deductibles - only very small co-pays for office visits and prescriptions. I've had three surgeries since 2007, all of which were 100% covered. I'm willing to pay more to keep my plan. What worries me is whether freeloaders will, even more than they do now, drain the system of resources such that even my employer will get rid of coverage or no longer offer plans that cover everything. I don't expect my medical needs to go down as I age, but people in my field don't retire early, and I can expect to be working for quite a while. Frankly, I'm hoping that I'll get to personally know so many doctors through my job that I'll have friends who can help me out informally if things go south even for those of us who are playing by the rules and paying into the system.
