A Physician on ObamaCare: Patients Ask Me to Break the Law

ObamaCare guarantees everyone an annual 'free exam' but that's it - and it would be illegal to ask about anything else during the same visit.

If you are here for that annual exam, you will not be covered if you want to discuss any new ailment or unstable condition. I cannot bait and switch to another code - that's illegal. We, the physicians, are audited all the time and can lose our license for insurance fraud.

You, the patient, will then have to make a decision.

Do you want your "free" yearly exam, or do you want to pay for a visit which is coded for a particular, new problem? You can have my "free" exam if you only discuss what Obamacare wants me to discuss.

...On top of all of that, doctors will be obligated - that's right, obligated - to talk to you about things you may have no interest or need to talk about.

You may just want to have a pap smear or check your cholesterol. However, I am now mandated by the government to talk to you about your weight, exercise, family life, smoking, sexual abuse(!), and even to ask if you wear seat belts. And I am mandated to record your answers.

I am a physician. But I need to tell you to wear a seat belt and then record your answer.

Elections have Consequences: No More Student Loan Deductions for You

It turns out that the Bush Tax Cuts For The RichTM also let a lot of people deduct their student loan interest. But that will be going away come Jan. 1st:

It appears that along with reducing taxes on rich people (those making more than $250,000 per year), allowing the Bush tax cuts to expire at the end of the year is going to have a bunch of other interesting effects, including: "The student loan interest deduction will be disallowed for hundreds of thousands of families." 

...Which means a lot of Obama voters -- people that bought into Obama's "soak the rich" rhetoric -- who have been able to deduct the first $2,500 of student loan interest from their federal income taxes -- are going to lose that deduction if they are more than five years into their loans.

Well if you bought the Obama ticket, then enjoy the Obama economic ride.

Poverty About to Skyrocket

The poverty rate is about to shoot up and permanently stay there. Why? Because it's going to be graded on the curve which guarantees there will always be an 'impoverished' 10%.

Now, a family of four with full medical insurance, living in Oakland, can be considered "poor" if its yearly pre-tax income is below $42,500. In Washington, DC, the figure is $40,300; in Boston, $39,500; in New York, $37,900.

Remarkably, for the first time these new poverty thresholds are linked to an "escalator" that will boost them faster than inflation year after year. The income thresholds will rise automatically in direct proportion to any rise in the actual living standards of the average American.

While the old poverty measure counted absolute purchasing power (how much steak and potatoes you can buy), the new measure counts comparative purchasing power (how much steak and potatoes you can buy relative to other people.)

Are the Five Stages of Grief All BS?

By now most people are familiar with the Kübler-Ross five stages of grief. But it turns out that there's very little evidence that it's true:

The idea that grief has specific stages is a popular belief and was given its most professional gloss by the Swiss psychiatrist Elisabeth Kübler-Ross, who is often cited as suggesting that mourners pass through stages of denial, anger, bargaining, depression, and acceptance. ...But regardless of how accurately her ideas were used, the evidence for these stages evaporates under scrutiny - perhaps unsurprising considering they were based on little more than casual observation and creative thinking.

In contrast, psychologist George Bonanno has studied the course of grief by following people from before they were bereaved to months and even years afterwards. It turns out that there is little evidence for a progression through specific stages of adjustment, and even the belief that most people are plunged into despair and gradually "get better" turns out to be little more than cliché. This is not to say that sadness isn't a common response to loss, but an experience of deep debilitating anguish tends to be the exception rather than the rule. In fact, two-thirds of people are resilient in the face of losing a loved one - in other words, they are sorrowful but they are neither depressed nor disabled by their experience.

5 Things Nobody Tells You About Living in Japan

Well my experience living in Japan almost two decades ago and visiting in the last few years pretty much matches what the Cracked writer says. Japan has a weird mix of high and low-tech - so people may walk around with gadgets more advanced than anything in the US yet most business is done with old-school faxes and involves quite a lot of paper-work. And public wi-fi and on-line forms for most services are extremely rare.

And then you have the national utter lack of decent heating systems in a country whose climate is essentially the same as the US eastern seaboard.

You can pick up an electric AC/heater, if you feel like throwing down hundreds of dollars to pay for both the unit and the required professional installation, but even then it's only enough to cover one room. The Japanese simply do not heat more than one or two places in the entire house -- they never have, and they aren't likely to start before you move here. Your only other option is a kerosene heater, which you can't really leave running overnight unless you're trying to burn and/or suffocate your family to death. Of course, having one also means keeping several cans of kerosene around the house at all times, so the "burning" thing may eventually happen on its own.

But Japanese blogger AMPONTAN disagrees.

64 I have already experienced the "you can't talk about your other medical issue during your physical." It wasn't the doctor who told me this. It was the chick working the appointment line on base. I asked her if she was going to follow me in and listen while I told the doctor what was wrong. I don't think she appreciated it very much, but I sure felt better. I love government run medicine. I was going to switch from Prime to the PPO, but it's going to be just like this everywhere else soon. I can't wait until private docs have to close every last Thursday of the month for mandatory training.

She's the only one that can save the party from the tea party extremists that now run it. Snowe has bravely decried the fierce partisanship that now rules D.C. 77 In case no one's mentioned it yet, the problem with the Kubler-Ross thing is that it *wasn't meant for* dealing with grief. It was meant for facing one's own impending death, not dealing with the death of someone else.

Polliwog, a friend recommended that I read that book because my mother had Alzheimer's at the time. I bought it and never read it. When my mother passed away, I was ready because she had suffered enough and she still knew me and my brother.

In my opinion whoever is the next RNC chair they need to have each candidate sign they will not run third party (Effin Johnson), institute a run off primary process and close all primaries. I'd break the primaries up into regions and each region has primaries two weeks apart. After each primary set any candidates under five percent are taken off the next sets ballot. We still would've ended up with Romney this time but only because of a weak field. This would also force republicans to expend time and energy in areas not named Iowa and New Hampshire. Part of our problem has been no candidate actually has to compete in 3/4 of the country and, given primary v. General rules, starts from behind in some very important states.

I enjoyed a couple of weeks in Japan a few years back. Any country that sells beer out of vending machines on the street is A-OK with me. Ice cold beer on-demand 24 hours a day - does it get any better than that?
Toilets fall into the high tech / low tech thing. They're either holes in the ground or an electronic masterpiece that squirts water up your butt to clean you up.
True about the language thing, too. Tourist places- like getting a rail card - nobody spoke english or could communicate. At a 7-11 though, the employees would strike a conversation in fluent english.

239 I've been to Japan about 5 times and eventually learned enough Japanese to get around. They think their language is nearly impossible for Gaijin (foreigners, but in practice, "pale faces"). It's not. It's not nearly as hard to learn to speak as German, BUT reading at least katakana and hiragana are important to easily navigate Tokyo's subway. Japan is fun if you approach it properly (it is quite bizarro land) and there is some spectacular scenery. I'm prefer Osaka to Tokyo, but Kyoto's quite nice.

He came as an "Emmitt" and that's is who he is. It really wouldn't be right to change it. He is kinda nicked up (he's been in a scrap/scrape, before), and he was pretty skitty when he arrived. But he seems to be over that. He's neutered, but he's an alpha. I think he realizes he's "a lucky dog", and he's settling in beautifully. He's high-spirited and attentaive. He's a keeper, and I think he's gonna' be a pretty good watch-dog.He's "mouth-feely" right now, and following Mrs. and the boys from room-to-room, faithfully.I'll give it thru the holidays before we "field and gun" trial test him. And that "my stupid mouth" is just an everyday thang.

Get one that is chambered for 5.56 and it will shoot .223 just fine. The cartridge itself is the same, the difference is the .223 has a SAAMI load and the 5.56 is military loaded, which isn't the same as SAAMI standards and is usually a bit hotter. The 5.56 upper will handle .223, the .223 may not handle the hotter 5.56 loads.

I hope I got that acronym right, but if I didn't I'm sure I will be corrected soon.

Heh. I started driving the car yesterday and when I got to the corner, the brakes were acting funny. By funny I mean, I had to put the brake all the way to the floor to get it to stop. I went to the dealer. Turns out the fluid was leaking out. Also, the back brakes were worn. I'm lucky I didn't bite the big one in a flaming crash.

Anyway, I went to the Chrysler Jeep dealer near my dad's. I'm feeling kind of charitable toward them because this was my only time away from Dad this week, when someone else was staying with him. I got there at 1:45 and they managed to squeeze me in and get the work done.

Also, this dealer has a really nice service lounge. They had free wireless for my Kindle fire, free homemade cookies, coffee, and nice comfy chairs. Even a fake fireplace. And they had a couple of computers that customers were using. They call it their internet cafe. This is so much nicer than the dealer near my own house that I'm thinking of getting all my work done there by arranging for someone to sit with my dad.

for simpicity you can shoot .223 in a 5.56 chamber, but it is reccomended not to shoot 5.56 in a .223 chamber

chambers don't expand, your face will if you blow the gun up, so don't do that.
And shooting with a full face mortorcycle helmet on is not 'cool'

I agree with you on all those points. regarding the weight point, if you are only planning on defending the home and doing so from inside it is probably better to go with the bigger round. I am a big fan of both but over time I have come to think of the 7.62 as my five iron.

If you're not using "careful shot placement" then you shouldn't be hunting in the first place. The key is under 150 yards for .223. I'm usually at 50-100 yards. Anything over 150 yards you should be using something in a caliber that at least begins in a 3.

A friend of mine was stationed a few times in A-stan and his duty entailed a bunch of cool stuff.

One thing he said that was memorable is he was on a Black Hawk with a group of Delta in 2009, and every Delta had a FN SCAR in 7.62

FWIW, a .223 with proper loads will do the job at 150 yards. You want something bigger for a longer range is all I'm saying. 30-30 or 30-06 or .308 all seem to do very well at the 200+ ranges.

No one can prepare for every single little thing. You will go crazy just to try.
You can have a compound made with containers stacked 20 feet high, you still have to drink, you still have to eat, you still have to get along with your neighbors.

I do a fair amount of hiking and can manage a fairly large pack no problem. I do realize I am not wearing all of the body armor and the circumstances. My point about the ammo is I would always feel better having more just in case. anything I could swap for ammo I would.

Yep. The best thing you can do to be prepared is be mentally prepared. Stocks of food and water sure, guns and ammo OK, but mostly you have to have the mindset that you are on your own and no one is going to come help you...what can you do to make the most of what you have available?


You can't be prepared for everything, but you can at least be prepared for the most likely scenarios (mostly that is natural disaster; storms, earthquakes, etc).

the 7-08 is a good round

any if anyone is hunting a quadraped, shoot the shoulder blade, the critter drops, rather than a heart/lung shot that the fucking critter runs like Forest Gump and you may never find it

My standard pack is 60 pounds for a hike of a week or more. As you noted, usually no rifle and ammo; if I have a rifle then not much ammo since nothing I am shooting at shoots back. I can do more and depending who I am dragging along with me I will carry more, especially if someone needs something more than a bivy to sleep in.

Er, not really. The fiscal cliff (sequestration + end Bush tax rates) only gets us about halfway to a balanced budget, from the most optimistic calculations I've seen. Assuming higher taxes and the deteriorating regulatory environment don't impact revenues (hint: they will and badly). Assuming the economy will remain in stable stagnation (hint: it won't-see the job report Friday or the weekly unemployment claims). Assuming Obamacare doesn't cost more to the federal government than projected and the fact that they double counted Medicare 'savings' twice doesn't affect the calculation (hint: ha ha ha ha).

The only real terms come if the debt limit isn't raised. Then we have an honest discussion. I doubt the GOP will do that because they don't want honesty any more than the dems do.

I've always said the R party doesn't always vote for conservatives, but it votes conservatively, and unfortunately that seems to coincide these days with "popular" and/or well-known.

I hate to be so much of a cynic, but I believe Christie knew what he was doing when he came out and licked Obama's kneecaps after Sandy.

In general, Democrats win low turnout elections, and Republicans win high turnout elections (in general). So it behooves Republicans to ensure a high turnout election. So how do we do that.

The one thing that neither McCain nor Romney would do was get into conflict with Obama. Certainly they had areas of disagreement, but they wouldn't actually get into conflict. Now, this is important because conflict is exciting. People get involved in conflict; they root for one side or the other.

Take, for example, American Idol. American Idol did better every year until Simon Cowell left. Why? Because Simon Cowell was a dick. He was very specifically a dick, because it caused conflict. With him gone, there is no conflict, and the show is boring.

Or take American Wrestling (or, as we called it, "wrastling."). There was a time when no one paid attention to it. It wasn't entertaining at all. The some guy came along and decided that his wrastling persona was going to be "the bad guy." Suddenly, it wasn't two grown men in their underwear tussling in a boxing ring, it was a fight between Good and Evil. To this day, the WWE keeps that dynamic, and they do very well.

I think Republican politicians, especially on the national stage, need to court a certain kind of controversy, of conflict. Certain "well duh" rules apply here of course (if you're ever having to explain that you think rape is bad, you've already lost, for instance). But imagine if, during one of the debates, Obama had said something about having to "raise revenue," and Romney had returned with this rejoinder:

"Mr. President, I completely reject your premise. We don't have a revenue problem, I'm not particularly interested in seeing the US Government take in more money. We have a spending problem, and that is what we should be attacking."

Now, instead of his mealy mouthed "Well, I want to raise revenues, too!" we have conflict. One side wants to "raise revenue," and the other wants to "cut spending." Now we have conflict. Now we have controversy. Now we have sides.

I see it as part of the classical argument related to intent (where it was clearly stated at the passage of the bill, in what little discussion there was, that it WAS NOT a tax), versus interpretation (where the accommodating traitor Roberts said Congress has unlimited power to tax, so I'll just tweak the "interpretation" to assume that Congress really meant to call it a tax and the TAX is therefore Constitutional).

637 Niedermayer,I can't speak for everyone,but the reason is because it was never called a tax during debate/voting in the House or Senate.As a matter of fact it was vehemently denied to be oneby many a Democrat,IIRC.
Well,lo and behold,it gets before the Court and you decide the only way it is Constitutional is to be deemed a tax?Fuck that shit.Unfortunately that motherfucker Roberts decided to "keep the peace" rather than send the Country into a Constitutional crisis.Because thats what it would have been.How would the Democrats have explained away thier vote for law that was deemed unConstitutional,especially considering the manner in which they passed it.
I decided then and there to LIB.The Government no longer has any restraints on it.They literally can do anything they fucking want to you and retroactively call it a tax.And this fucking jackwagon just set the precedent for it.Nope LET IT FUCKING BURN!

Although the argument was changed before the court from what was presented in Congress, did SCOTUS have leave to evaluate the conditions leading up to the argument or only the argument as presented before the court?

Was the court permitted to say "HEY! That's not what you said when the law was passed!"?

640 Nobody is going to read this, but in defense of Kunler Ross, I have the original paperback of On Death and Dying. She took great pains to say that not everyone goes through all five stages or in any particular order, though our pop culture has latched onto that misconception. Her book was meant as a way to grant humanity and dignity to the dying and improve bedside manner, as well as to communicate with the bereaved. It's a very good book and highly recommended.

Rubio would be my first choice. Sandoval is not bad,either. Palin should challenge Begich in AK. Walker would be great.

Realistically, the 2016 GOP ticket is Rubio / Bush. It will take a Bush on the ticket to challenge a Clinton. Americans love their dynasties, but they also love the new hot thing. Dems don't have anything near another Obama, but Republicans have Rubio. They will add a Bush for security, but the media will talk about the "tension" between the rising star and his mentor during the primaries.
