Yes, Obama Certainly Did Cut Medicare, Says Public Trustee For Medicare Fund

Apparently we're still debating whether you can spend the same $716 billion twice.

Perhaps the easiest way to understand this is to look at Social Security. If we generate $1 in savings within that program, then that's $1 that Social Security can spend later. If we also claimed this same $1 to finance a new spending program, we would clearly be adding to the total federal deficit. There has long been bipartisan understanding of this aspect of Social Security, which is why Congress's paygo rules prohibit using Social Security savings as an offset to pay for unrelated federal spending.

No such prohibition exists in the budget process against committing Medicare savings simultaneously to Medicare and to pay for a new federal program. It's this budget loophole, unique to Medicare, that gives the health law's spending constraints and payroll tax hikes the appearance of reducing federal deficits. But it is appearance, not reality. If you have only $1 of income and are obliged to pay a dollar each to two different recipients, then you will have to borrow another $1. This is effectively what the health law does. It authorizes far more in spending than it creates in savings.

How much more? Charles Blahous's study, "The Fiscal Consequences of the Affordable Care Act," published last month by the Mercatus Center, found that the health law would add over $340 billion to federal deficits over the next 10 years. Over the longer term, deficits would run into the trillions.

We are at the point where the guy needs to use an analogy to explain why you can't spend the same dollar twice. He actually has to tease this chain of reasoning out for people.

And yet, he must, because Obama's Media Palace Guard is attempting to claim otherwise.

MYTH THREE: OBAMACARE SLASHES $700 BLN FROM MEDICARE

Facts: The Romney-Ryan campaign has trotted out this scary-sounding number to deflect attention from Ryan's voucher plan. But it is largely a false claim because it implies that the health reform law slashes benefits.

The Affordable Care Act actually delivers expanded benefits to seniors. It closes the prescription drug donut hole over time, with 3.6 million seniors saving a collective $2.1 billion last year; it also expands preventive services, including an annual wellness visit, mammograms and prostate cancer screenings with no out-of-pocket cost.

Obamacare does cut $700 billion in Medicare spending over a 10-year period. But the cuts are adjustments in payments to Medicare providers, which are mostly meaningless to patients. According to the CBO, the ACA's 10-year cuts include $415 billion in fee-for-service payments to healthcare providers, $156 billion in reduced payments to Medicare Advantage plans, $56 billion to hospitals, and $114 billion in other miscellaneous cuts far too numerous to detail here.

That article -- from Reuters, natch -- is worth reading in its entirety. It is breathtaking. It is simply an Obama press release, and not a word of it is true.

And it further contends that cutting $718 billion from the program will, for the first time in the history of history, not impact beneficiaries at all.

Amazing. Every time Republicans propose a cut it's going to kill people. But when Democrats cut $718 billion, it actually "shores up the system."

This is the sort of claim that could only be made by the borderline-retarded.

Certainly it is, at least on a philosophical level. But, unless ACA changed this (and I haven't heard that it did) Doctors' participation in Medicare is voluntary. They can fill out a fairly simple form (I've seen it) and then send a letter to all their Medicare patients saying they'll no longer be participating, and 90 days later (I think. Maybe 6 mos) they're out.

Then, just like seeing a doctor who isn't participating with your private insurance, they can bill you the full amount and make you take it up with Medicare. And even if they don't do that, they won't have a contract with Medicare preventing them from balance-billing (that is: billing more than the 'allowed' amount).

Yeah. Except for the whole bit where doctors will stop taking Medicaid at all because they're getting paid .0003 per hour for their work.

Oh, hey, here's something else. At least in PA, medical charges for treatment related to injuries sustained in auto accidents are linked to the Medicare reimbursement rates. If those rates are cut, so is the amount that doctors are allowed to charge under Act 6 for all treatment for auto related accidents. That means the providers are going to be making that up by increasing charges to others. So, let's presume I still lived in PA. If I go to the doctor for whatever, my costs will go up because the doctor can only charge .10 for the appointment for follow up care for the guy who got rearended because the Medicare reimbursement rates went down. So I, who do not receive Medicare, will be getting it from all sides since I'll be paying the taxes for the Medicare benefits I'll never see and also paying more for my own health care since the doctors are getting less from multiple other sources.

My mother had to spend a brief time in the hospital just this past week. One of the nurses tending to her -- a very nice young man apparently -- was on the mend himself from some kind of surgery. It was one of those "it's elective now, but eventually it will be necessary" kind of surgeries; not sure exactly what. Anyway, he told her, "Yeah, I wanted to get it out of the way this year, before the real shit hits the fan with the new insurance regs NEXT year."

It's going to be really, really bad.

PS - Oh, and the TV in the waiting room was tuned to Fox News.

We might have more doctors willing to accept Medicare.
My husband has not accepted new patients on Medicare for years but continues to care for existing patients, while he loses money on every one.

Docs have to sign a contract that they will accept whatever the plan pays.

Get healthy, stay healthy. Don't count on living an unhealthy lifestyle now and having state of the art medicine fix you up later. Bad gamble.

Not sure how it works with Tricare (not 100% sure about Medicare, but I'm on better footing there), but you should be able to go anyway. What will happen is they'll charge you up front (you can probably work out both a cash discount and a payment plan, by the way), and then you can send the bill Tricare.

Tricare will only reimburse you according to their "reasonable and customary" rates, but it's better than nothing.

Again- not sure it works that way with Tricare, but I'm pretty sure it should.

Okay, yet more story time, I think I've brought this up before. Old Firm had a client that was on a train that got hit by another train. Every four months, like clockwork, she got a notice from her health insurance company that they were denying the claim because her auto insurance was primarily responsible. I would then call the company and explain that, no, it wasn't, because she was not in an automobile at the time of the accident, she was on a train that was hit by another train.

Client gets letter, calls me, I call and the lady on the other end starts out all snotty with you know that her auto insurance is primary and I very patiently explain that, no, it's not because no automobile was involved since train A hit train B. We go through this a couple of times and she finally gets it. There's silence then she blurts out "I don't have a box to check for that!" I cracked up.


On the Act 6 thing, part of the statute prohibits doctors from balance billing. Providers do it anyway. I've gotten into several very nasty arguments with providers about this, to the point that I had a stack of the PA Supreme Court decision saying the law was valid on my desk to have handy to fax out. I would love to know if anyone has done a study to show what other payments will be affected by the Medicare reimbursement rate cuts.

I read an article in a medical publication recently about two orthopedic surgeons who would not accept Medicare.
BTW, this specialty has very high malpractice insurance rates.
They were the only two specialists in a certain geographic area, and were informed by the govt that they HAD to accept Medicare.
I'm still wondering, are the feds going to zip tie (can't say "chain") them to operating tables and make them work?

That whole thing was a clusterfuck of SCOAMF proportions. I ended up on a conference call with what was probably everyone in the health insurers office going "Yes. A water buffalo. Yes. In Pennsylvania. Yes, I know that's absurd. Yes, we have pictures of the water buffalo." IIRC, they lumped it in with the deer hits.

These things are always losing propositions from the start. If you set payments at more than the market rate then you would save money by not having Medicare at all. If you set payments lower than market rate then you get shortages and you would be better off without Medicare at all. If you set the payments at exactly the same rate as the market would then what the hell is the difference? There is no scenario in which you are better off with Medicare than without it until you start talking about robbing one group to buy votes from another. That is the only context in which any social program makes sense.

A few billion years of evolution and trillions of dollars of debt and we still don't understand the basics...

I asked if I could have my records on a thumb drive and not on their system and he said "no". I asked if I could opt out and he said "no". Since when is there no way to opt out?

He also said that everyone isn't using the same system so he doesn't know how it will work if say the cardiologist needs to send the GP records, they are on two different systems owned by two different companies. He said the cardiologist is with the company all the cardiologists went with and the GP is with the company all the GP's went with.

We also discussed the fact that every hospital is now part of the same network. So you go for a second opinion and the guy/gal would have to go against someone working in the same hospital corporation and it is uncomfortable as they can easily find out who gave the first opinion. There aren't many private hospitals, not in a network, left. My GP is not happy with the coming system which he says has been being put in place with the advent of these big hospital corporations.

It was funny, he said you thought you had to worry about "too big to fail" with banks, how about worrying about "too big to fail with hospital corporations".

Obama took 715 billion dollars out of the current program for seniors, to pay for a future program for everyone else.

Romney/Ryan's plan is to restore that money into Medicare.

Ryan's plan leaves current Medicare intact for those currently on it, and those up to ten years out (age 55 and up)

People under 55 can choose either to stay in Medicare, or opt out for private insurance, by way of a voucher program.

Number 1 - Do I have this correct?

Number 2 - How does this harm seniors, as the Dems claim?

Number 3 - Why is this a scary plan? It seems very not-scary to me. In fact it seems somewhat tame and rather sensible. Assuming I have it correct.

In 2004, OB/GYN malpractice insurance cost my husband $90,000.00. Last year, he didn't earn that much.

I do this for a living... I implement Medical Record systems for Doctors... and its a mess.
The Government mandated everything to go onto a system, without mandating any system to trade that information.... so the medical benefits we'd get from the system will not happen. The ONLY thing that gets better, is easier billing codes for Medicare and such... as that part IS standardized.

We lit off a 5 doctor offices new system 2 weeks ago... it currently takes 6 times longer to see a patient than it used to with paper records, as they have to input all the history information into the system.... and find there way around to what they want.... it will get better... but that is hundreds of man hours a week NOT spend on patients currently.

But if they DON'T use it? They get hit with a penalty on Medicare reimbursement rates (1% this year, and escalates in later years).

Tell me about it. My wife went in for her annual visit a month ago. The nurse looked at her with a puzzled look on her face as apparently her medical "record" showed her to be 5'11", 205 lbs with a knee that had been replaced two years ago. Oh,and herpes. Somehow someone else's medical record had been placed under her name and there was no way to delete it from the system, so the nurse had to go back in and add a new record stating that the previous information was in error in order to avoid insurance problems in the future. Which, incidentally, I believe will almost certainly occur because, you know, Murphy's Law and all of that.

Makes me even more warm and fuzzy about this new law.

How then to explain my colleague received TODAY from her doctor. The MD, as a result of "ongoing changes in our healthcare system" informed her that getting quick appointments will ONLY be available if my colleague joins a $2000 a year "personalized health care program." Otherwise, you're on a wait list to see the doc...apparently a long one.

The MD is deliberately shrinking her patient count, particularly Medicare patients, and clearly, instead of dealing with the government hassles and poor reimbursements, is effectively setting up a special club for those who can afford it. And I'm hearing that more and more docs are doing this is direct response to Obamacare and Medicare theft.

The $2000, by the way, is on TOP of what my colleague pays in employer provided health benefits...right out of her pocket.

My colleague is a wavering liberal. I said to her, THIS is why you should really consider who you are voting for.

I'm workin' on it!

Mostly meaningless? That fits right in with the entire Obama presidency if true. But somehow I think it will be mostly meaningless to attempt to find a Medicare provider that will accept you as a patient if ObamaCare refuses to pay the bill. Cut payments to providers and providers will quit providing. But that was the plan from the beginning,wasn't it?
